We’ve seen first hand the effect dementia can have on both a person and their loved ones. We provide personalised dementia care that respects each individual’s story, values their experiences, and supports them to live with dignity and comfort.

Understanding dementia care

Dementia affects more than memory, it can influence mood, communication, and generally the ability to manage daily life.

Our role is not only to provide clinical support, but also to preserve identity and dignity. We create familiar routines, adapt the home environment, and encourage activities that bring joy and connection.

How our dementia care works

We take time to understand each person - their routines, preferences, and the life experiences that shape who they are. Our care is guided by the National Institute for Health and Care Excellence (NICE) recommendations on person-centred support.

We focus on:

  • Recognising individual worth - Valuing every person regardless of age or cognitive ability.
  • Respecting identity - Tailoring care around personality, routines, and strategies to promote memory.
  • Listening to voices - Giving people with dementia a say in their own care.
  • Supporting connections - Encouraging family involvement and maintaining social relationships.

Care Connect is our 24/7 telecare service that keeps individuals connected to professional help at the touch of a button.

Care Connect provides:

  • Fall detection and GPS tracking - Alerts us if someone wanders or needs urgent help.
  • Direct connection to our nurses - Immediate support at the touch of a button.
  • Independence at home - Enabling people with dementia to stay in familiar surroundings for longer.

All clients who receive care and support from Care Hub are automatically eligible for Care Connect, our Alarm service (if required). This also includes 24-hour nurse response and health checks.
